Providing Specialized Support for Patients and Families

Harbor Village Rehabilitation & Nursing Center is dedicated to supporting the unique needs of patients with dementia and Alzheimer’s diseases.

 

  • One of our key employees was a recipient for the Alzheimer’s Association’s 2018 Woman’s Champion Award for her significant support and dedication to the fight against Alzheimer’s.
  • Several of our staff have been recipients of the Annual caregiver of the year award from the CT Alzheimer’s association.

Through our Special Care Program individuals can benefit from:
  • Experience: Our nursing and rehabilitation center has provided care to our patients on the special care dementia unit since its inception in 1983. That’s 35 years of experience. 
  • Advanced clinical expertise and training: Utilizing our standardized training Program, staff initially receive over eight hours of skills training in models of assessment, approach and patient care techniques. Continuing education is provided to enhance each resident’s independence, functional status and comfort level.
  • A balanced approach: Our goal is to meet the social, psychological, physical, and medical needs of our patients, with family involvement. Our specialized program is designed to create a positive patient specific experience. Our behavior team meetings focus on nonpharmacological interventions and proactive approaches to care and behaviors.
Through these efforts, families may see a reduction in distressing behavior problems that are often present, thus enhancing family interaction and enriching patients’ quality of life. Combining all these methods, and collaborating with Hospice dementia specific end of life care providers, we serve patients experiencing memory decline or other cognitive, behavioral and medical complications through the continuum of care.
