Harbor Village Rehabilitation & Nursing Center is focused on simplifying the admission process to our facility and reducing unnecessary hospital admissions and readmissions.
Are You Or Your Loved One:
- Requiring more frequent physician visits
- Noticing rapid changes in condition
- Experiencing wounds that are not healing
- Not thriving at home or other community setting
- Requiring increased assistance and daily care
- Having difficulty recovering after a recent hospital stay
- Experiencing falls
No Need to Go Back to the Emergency Room
- We work directly with your Physician or Home Care Provider
- We will verify all insurance and research coverage availability
- We have reduced respite rates
When the time comes to return home:
- On-site case management and social services to assist with discharge planning and allocation of resources
- Home evaluations and safety assessments by our therapy team
- Medication management and education for residents and family to ensure safe administration
- Collaboration with home care agencies, senior resources and caregivers
- Schedule post-discharge Physician follow-ups and provide Interdisciplinary discharge instructions
